fix: Windows ExecutionPolicy error — set Bypass + remove .ps1 wrapper

npm creates claude.ps1 wrapper which gets blocked by default PS
ExecutionPolicy. Fix: set Bypass for CurrentUser and remove the .ps1
wrapper (claude.cmd still works fine).
